There's also the Start Menu for AllUsers.

There's a lot of places that it can hide in the Registry, but you say
you've searched them.

Sorry, I'm afraid my malware-removal skills have atrophied since I
don't primarily run nor support Windows any more. It's weird that
Googling produces _zero_ results.
