figure out what is going on.

And, check for updates often...  It is already been updated to version
0.9.022 since the initial release due to some bug fixes I've made.

Just downloaded it.  Only thing I can verify is that it runs and the form
appears.

I'm looking at it to see if it presents a better way to do report dialogs;
e.g., when a user calls for a report, we have to ask for which month, how to
sort it, subtotals or not, etc. etc.  Til now, these have been designed
individually. DynamicForm might be a cheaper and more robust way to do it.
Will let you know how I make out.

BTW, first thing I did was separate the sample driver from the class
definitions, to avoid screwing up your code while developing mine.
