Paying 'taxes' and filing reports throughout the year is about the biggest downside. Quarterly state unemployment reports, Fed withholding remittance monthly and quarterly reports, and annual Fed Unemployment report...don't miss 'em or it's a big pain. A good calendar app with reminder pop-up or email notices...well worth the time to set up!

The annual cost of Quickbook's cheapest payroll option is now up to $300+ (plus the price of Quickbooks on the two-year forced upgrade plan) but doing payroll manually is a bigger pain for me.
