Joe,

I usually blame it on a random mouse click. A user grabs the mouse, pressing
the button, and moving the mouse around to find the pointer, all while
moving something to a place it wasn't meant to be. Many times when helping
customers that have lost a file or folder, I have found the missing item in
a folder under where it should have been. I explain the random mouse click
and hear an "oh?".
