Michael,
Sounds to me like the custom formsize is slightly larger than the page size
set up on the printer as it must be slightly smaller or exactly the same.
 
1. Make sure the "skip top of form" setting is set to NO in the hardware
Printer setup (Shift and Menu whilst offline to enter setup menu)

2. Ensure that the custom pagesize in excel is smaller than the papersize in
the printer (again you can set this up on the printer)

3. Adjust the "microfeed" setting on the printer frontpanel so that
consecutive manua PageFeeds are EXACTLY aligned to top of form.

Got a colleague who's got a client with an Excel printing problem.  
Client has a Oki Microline 591 24-pin printer.  Form is a custom form 
size (don't have dimensions at hand) but he said it's "jumping" every 
print slightly, and eventually gets out of alignment (vertically) by the 
4th or 5th print?  I believe he had set the custom size setting in Excel 
but apparently it didn't work as he expected.

Ideas?
