I think you might want to reconsider "normalization". That they have a code and a description is not the perspective to use. What they are is what decides where they belong, not the data structure. When does the OTLT break? When there is a Salutation entry for "Dr.", "Doctor" and another set of records indicating Employee types, "Dr.", "Doctor". Despite having a separate Type field, the fact remains there are now 2 records for "Doctor" in this table, which is AFAIK against the concept of normalization.
