Line 12 (4) "Cost Sharing-The term "cost-sharing" includes deductibles, 
coinsurance, copayments, and similar charges, but does not include 
premiums, balance billing amounts for non-network providers, or spending 
for non-covered services." Add all that up and you may as well have no 
plan at all. On cost alone this flunks!
