Assuming they can do their work which involves connecting to various hospitals via VPN, then nobody needs the Internet except for the managers.
What I think we're going to do is set up a computer or two just for surfing with no connections to the company's data and it will be isolated from the other computers. That way people on their lunch break can do anything they want.
