I really don't know how to deal with that.  I don't think proj should do
something that isn't technically justifiable to make people that don't
really understand think things are ok.  I think that leaves us as
"that's metadata about how you obtained heights in that vertical CRS,
sort of in the same category as which RTN you used".

I would think that if surveyors are using HAE and geoid models to obtain
orthometric heights, they really need to understand that, and the
accuracy issues vs leveled heights from passive controls.  I would think
that either the RPLS exam and continuing education requirements would
ensure this, or they shouldn't be using HAE and geoid models.  But
IANARPLS :-)
