Resource temporarily unavailable may indicate that the site was busy at
the moment bibindex tried.  This type of errors can be usually ignored
if things work after a retry.

This type of errors indicate that the link between the records and the
attached files was somehow broken.  Looking at this record, it seems
that your MARCXML format for this record is fine, but your MARC format
is not, compare the following two URLs:

   good: <http://documents.ct.infn.it/record/343/export/xm>
   bad: <http://documents.ct.infn.it/record/343/export/hm>

This may indicate that your bibfmt table is OK, but your bibxxx tables
are somehow now populated.  Have you touched your bibliographic tables
in any way?  Can you have a look at say bibrec, bibrec_bib24x, and
bib24x tables to see what is stored there?  Are they empty?  If so, how
this may have happened?  (In the worst of cases you will have to
regenerate your bibxxx tables from MARCXML.)

P.S. The link between the record and the attached files is stored in the
     bibrec, bibrec_bibdoc, and bibdoc tables; you may want to check the
     contents of those too.
