#250: Issues with bibdocfile

There are some crucial improvements needed in bibdocfile to make it more
usable and scalable:
* It is very slow, for instance deleting one file takes about 2 minutes on
INSPIRE dev (just to submit the task to bibsched) and it uses heavily CPU
at that time
* the --set-docname option is ignored when used with the --append option
In addition, for the examples section of the doc: there is no --format
option
