#810: Fix CV Format Linking

This consists in two parts:
1) Re-order items in output format dropdown menu, so that all CV formats
are together, e.g.:
* CV format (LaTeX)
* CV format (html)
* CV format (text only)
2) Add link to CV format to the author pages.
