In answer to your first question for things such as fiduials, tooling holes,
mounting holes, etc, I have a set of schematic symbols for these process
related parts and place them on the *.PRJ sheet.  I use the project sheet
for CADing purposes - "for office use only" - and it never goes beyond my
PC.  This project sheet also contains the sheet symbol of sheet 1 (tying the
project together) which then becomes the top sheet of the "public"
heirarchy.  These items obviously appear on BOM outputs and I assign them
designators with a leading 0 (zero) so they are grouped and therefore easily
found.

I don't think there is any tidy answer to your second question regarding
exclusion from synchronisation.  In one case where I have wanted to use this
I just left the footprint field blank and lived with the expected 'errors'
generated by the synchroniser.
