Comment(by coling): Can you attach the output from {{{pacmd list}}}? Also can you start PA manually via command line with {{{pulseaudio -vvvv}}} and attach the debug output produced too? (you may have to disable autospawn and kill the currently running PA first {{{echo "autospawn=no" >~/.pulse/client.conf; pulseaudio -k}}} As a potential quick fix, you could perhaps try setting the {{{default- sample-rate = 48000}}} (probably best copy /etc/pulse/daemon.conf to ~/.pulse/daemon.conf and make the change there) as perhaps there is some problem opening the HDMI with the default 44.1 when probing.
