I found the 'Client certificate verification' section at the bottom of 
http://projects.puppetlabs.com/projects/puppet/wiki/Using_Mongrel and that 
solved the problem. 

I think that whole set of pages needs to be re-written into proper 
documentation as it's a mess, the "Using Mongrel On Enterprise Linux" didn't 
have any mention of the client certificate verification problem. 

It can be re-written properly and just have a section at the bottom which lists 
the different paths and package names for each different Linux distro or Unix 
variant.

I've setup puppet to use mongrel behind an Apache load balancer according to 
the documentation on your wiki:

http://projects.puppetlabs.com/projects/puppet/wiki/Using_Mongrel_On_Enterprise_Linux

It works for existing clients, however trying to add a new client I get:
    puppetd --test -v --waitforcert 60
    warning: peer certificate won't be verified in this SSL session
    err: Could not request certificate: sslv3 alert handshake failure
