That sounds really like a bad crash... 

For a quick fix you could simply use the original ComSwiki for
Windows (based on an older Squeak Version). 
This is available at

 http://minnow.cc.gatech.edu/swiki/10 (with win-wm)  or
 http://minnow.cc.gatech.edu/swiki/32 (only image+files)

With these you should get a running Swiki soon.

But it would of course be interesting to track that bug
down... the swiki and Comanche - packages on SqueakMap
should contain the same code as the "original" ComSwiki
from http://minnow.cc.gatech.edu/swiki/

To narrow the possible causes, you could install swiki
via the sources from http://minnow.cc.gatech.edu/swiki/39.

If this works, we'd know that something with the .SAR version
from SqueakMap isn't correct (or, I guess, that maybe we run
into a bug of the win VM on Win98 while using the SARInstaller...)
