Apart from moving the spinner/progress bar/bootstrap loader image from
index.html to the bootstrap script I think we're ready to upload. I'd
like to cope with this later in a clean way (as mentioned in another
thread, "Ajax spinner ..." [1]). It doesn't disturb visually and
functionally. (The maximum recursion error still pops up with pyjd on
my system, that's what we should check on, but that's about it.)

Can we upload the as-is state from pyjs_site, book, and examples now?
I'd love to see this online before February ends.
