The first part (creating the configuration) was documented but not the
second (setting up ConfigMiddleware).  I never noticed it until now
because this was the first time a non-default config setting was
critical.

That sounds like a good safety measure, and would also free up Pylons
to make API changes without breaking existing sites.

Quixote maintains both a changelog and an UPGRADING document, which
tells how applications should (or can) change with each framework
version.  We should probably do the same.
