I never tried it, but localhost should work. If you right click on --- OSM --- 
you can setup own URLs. And with latest version you have to right click in the 
"stream" tab to add a new URL to the "stream" map category.

Yes, so far the tiles are erased after 8 days. The latest changes to the TMS 
support use a Qt internal disc cache. I have no clue how long this cache stores 
the tiles.

The time limit has two reasons: Especially OSM maps change frequently and users 
want to see the changes after a time. And of course the TMS servers' terms of 
use generally allow transient caching of the tiles, only.

But afaik MOBAC helps you to convert a tile cache into something QLGT 
understands.
