Hi Peter, I am not sure whether someone has already answered you but in the case not yet, here's what works for me.
First, I've compiled QLGT 1.4 from source (openSuSE 12.1, after adding all requirements the compilation went without a single complaint :-). Assuming the maps are e.g. in ~/david/maps/zoom/x/y.png (zoom, x, and y are numbers describing the tiles), you go to the "Maps" tab in QLGT, select the "Stream" tab, right-click, "Add TMS map", and in the "URL" field fill "file://home/david/maps/%3/%2/%1.png".
