No - composing the contents of the index is off course manual work - I collect index references to each entry in the complete TOC and save this in a "basis file". In the next step I use a script to generate the index in the given format. The "basis file" and the script will be published later after same "trial period". The use of the script implies to re-run your "make nav" because my scripts do not insert the navbar info. This requirement should be inserted later in the handling rules for the index creation.
I could whip-up a small script
(mostly stolen from "LinkCheck.sh") which would produce a complete list
of local fraction identifiers (the "...#...' thingies) that are not yet
referred to in both, "AxAdvIndex.md" and a sort of exclude list contain-
ing any links not relevant for the index.
This would give a list of things that could be indexed at all.

Thanks for this proposal but I have already done the same thing. The main problem is to decide what and how to include in the index. And this is manual decision. Your proposal (and mine) helps to do this.
Regarding the format of the index file, I think a table is not the best
approach. I'm tending to definition lists, and if time permits, I will
put something online these days in the playground. Stay tuned.

The MD format is very limited with regard to formatting options. A definition list as used in HTML would certainly be nicer. Do you know a MD formatting for such a list? Input should be in MD form and Bitbucket MD doesn't allow HTML inclusions. A format change wouldn't be a big thing. It is more or less a change of a format statement.
