I use QMAILQUEUE with qmail-qfilter, and the deny-filetypes perl script
included with qmail-qfilter on one machine.

I use MessageWall on another machine which does much more filtering than
qmail-smtpd with patches. It also does virus filtering (but most of the
viruses are bounced by the attachment filter).

I think MessageWall in-front of qmail-smtpd is better since qmail-smtpd
doesn't provide as many filtering capabilities. It does need lots of
preallocated RAM though :(.
