Hmmm, they guess? They scan the networks? They use
footprint patterns? They have very good spies? :)
Probably what they do is crawl the web for e-mail
address and try to connect to the domains to get open relay
servers, IMHO, they probably do that on the same network of
a compromised or weak server.

In Brazil we got some spammers using modified versions
of MTAs (so far qmail and sendmail), but you can use just a
part of the SMTP protocol, just to open the connection and do
the delivery, for sure there are tools, that try to workaround
throttle and other anti-spam techniques.
