Yes, I did run the patch rpm. Is there something else I should do on top of
that? Also what do I need to add to the top of my  run
/service/qmail-smtpd/run?

The QMAILQUEUE is an enviroment var.  You need to add the following to the
top of your run script in /service/qmail-smtpd (if that's what it's called
on your system.)  You DID mention that you are running daemontools so you
should have a /service/qmail-smtpd...

I'm also going to assume you've got a patched version of qmail queue so that
it "sees" this new env. var.

I hope this helps a little.  Sorry, I was pressed for time.  Maybe someone
else will answer in greater detail if this doesn't do it.
