I got my qmail-scanner to start quarantining spamassassin
tagged email by adding just two lines to the qmail-scanner-queue.pl:

$quarantine_event="Questionable content";
$quarantine_description="Content blocked by SpamAssassin testing.";

I put those right after:

$sa_comment = "Yes, hits=$sa_score required=$sa_max" if ($spamc_options =~
/\-c/);

Since you'll only reach that else clause if the spamassassin score
was not zero and above the threshold.
