Then restarted with:
csh -cf '/var/qmail/rc &'
/usr/local/bin/tcpserver -R -x/etc/tcp.smtp.cdb -c100 -u502 -g500
0 smtp \
/var/qmail/bin/qmail-smtpd &
It's running on a Redhat 6.0 box. It hasn't happened again thankfully, but it's not a production server and has a very small load... What would be a recommended procedure for diagnosing the exact problem the next time it occurs?

Exactly what OS are you running? How often does the hang occur? When the
hang occurs, can you kill -ALRM qmail-send or kill -TERM qmail-send? (You
said you killed it, right? kill -9 or what?) What happens if you kill -9
