1)  You didn't setup your control files properly (/var/qmail/control/*)

Read the INSTALL.ctl file (its in your build directory).  Did you run the
config script ?

2) Yes.  Read the INSTALL.maildir file (its in your build directory).  It
explains maildir vs. mbox.
