And have you checked to see if there are actually messages in the
appropriate maildir?

The qmail-send logs will tell you exactly where each delivery went.
If you want help from the list, tell us exactly how you have qmail
configured -- i.e. copies of all your start/service scripts, all patches
installed, what other packages you're using -- and what is happening,
i.e. logs.
