Is there a reason why you can't relay outgoing mail through your ISP's
mailserver? qmail can be configured to do this trivially, and without
any real loss of functionality (except control of the queue
configuration).
At one time, back when the mail-abuse.net stuff was free, you could
block *A LOT* of SPAM by using the DUL, RSS, and RBL. DUL stopped
people from connecting directly to your mail server, RSS stopped them
from sending through open relays, and the RBL stopped spammer-friendly
ISPs. It certainly raised the bar for spammers, so that a home user
wouldn't be so tempted to send out a typical chain-letter or "Make
Money Fast!" SPAM.
It wasn't perfect, but it was much better than with no SPAM
protection.
