Disk full or out of inodes, or permissions/ownership of your queue
structure or qmail-queue binary is incorrect.

You didn't look hard enough. There's lots of cases of this in the qmail
list archives, which are referenced from qmail.org.
To fix:
1) Stop qmail.
2) Run "make setup check" from the qmail source directory
3) Restart qmail.
If you still get similar errors in the logs after this, consider using
one of the queue repair tools listed at qmail.org.
