If rcpthosts is populated, and the only way you're allowing relay is via
the relaymailfrom patch, odds are that's how they're spamming. That
patch always sounded like a horrendously bad idea to me. No idea how
it's configured, so I can't really recommend anything other than killing
that patch and its control files, and basing relay on IP address or
POP-before-SMTP. :)
mail-from is _so_ trivial to forge -- IMHO noone should install that
patch in a public environment. The author, Chris Johnson, seems to feel
the same way...:
