This is becoming quite obvious that it is a problem with QmailAdmin.
Since I'm new to the list, I'm not sure if the developers actually read it
but if they do, I would appreciate any hints or tips on how to fix or work
around this bug.
I'm pretty sure you'll find that it's a problem with your installation, and not with qmailadmin. A lot of other people have been using 1.0.6 without any difficulty, and this is the first time I've seen a report like yours.
I just took a look at <http://www.1osc.com/cgi-bin/qmailadmin> and it didn't load any of the graphics. I went to <http://www.1osc.com/images/qmailadmin/> and got a 404 error.
Please try logging into qmailadmin and then going to the user list (where it isn't showing the delete.png graphic). Then send me the last 100 lines or so from your access_log and error_log on the server, along with a directory listing of your /images/qmailadmin.
Also, grep your httpd.conf file for "images" and "qmailadmin" to make sure you aren't aliasing the directory to some other location.
