I can recommend the qmail-spamc program that comes with SpamAssassin. It's a replacement for qmail-queue that scans all incoming mail and adds spam headers. You lose the ability to have custom prefs for each user, but it's a simpler way to have site-wide spam scanning (including messagaes forwarded off the server).
