[qmailadmin] Problem Creating Forwards

hello list,

i am facing problem adding forwards using qmailadmin. all other
features for accounts, mail robots, mailing lists are working fine

when i click on New Forward, i get the following error

Server error!

    The server encountered an internal error and was unable to
complete your request.

    Error message:
    Premature end of script headers: qmailadmin 

    If you think this is a server error, please contact the webmaster 

Error 500

my configuration is 
1. qmail 1.03
2. autoresponder-2.0.5
3. vpopmail-5.4.7
4. qmailadmin-1.2.3
5. ezmlm-0.53
am using postgresql as my backend (things are working fine, no problem
with that)

qmailamin permissions:
