If it was me, I'd do a fresh install of CentOS 4.0, following the guide on the qmailtoaster.com home page, and continue using the guide to install the toaster packages. I experienced problems when trying to install using CentOS 4.2, but a recent email to the list from Nick suggests that there is a switch that can be used to make it work on 4.2, although I'm not sure at what point the switch should be invoked.
