In response to #2 below, I recall using something called (I think) perfect maildir a verrry long time ago. I just googled it and came back with: http://home.uninet.ee/~ragnar/2md/ If this is the same script, I seem to recall running it one mbox at a time, and I think the main issue I experienced was that the date and times on the messages were not preserved in the conversion, but the script may have improved since. Obviously your mileage may vary.

Erik also has a link to a script: http://www.kabewm.com/dl/qmail//maildir.pl which may prove to be better.

Regarding #3:
- I found the 4.0 CentOS install to work nicely, but see my last post re: 4.2. - I had to add a trailing slash to the admin-toaster alias in /etc/httpd/conf/toaster.conf (no biggie)
 - I had to install mod_ssl to enable SSL access to the admin controls

All in all it was an easy install, and so far I'm very pleased with the project, its authors and the supportive community.
