The problem is that it is not consistent so when I try sending the same
email I don't get this error

That looks like there's too many recipients in the message. If you send
to more than 15 people at a time, you will get this error message. This
can be changed in your /etc/tcprules.d/tcp.smtp file, and then run
qmailctl cdb, and then reboot or qmailctl restart.
