If the mail is addressed to the exchange server you shouldn't have to
add artificial routes. I don't think I fully understand what you are
trying to accomplish. Perhaps you can explain the scenario a little
more clearly.

IE if you have a server cert set then TLS just works when both ends support
it.
If you want to route all outgoing mail via Exchange just put
':exchangeIP' in a txt file called /var/qmail/control/smtproutes.
