I still have a problem with some emails being rejected with
"<<< 554 mail server permanently rejected message (#5.3.0)" error, and I
can't figure out where they're being rejected.

I looked at all other /var/log/qmail logs, and can't find any indication
of where it's being rejected. The smtp log shows no indicatation of
rejection. It appears to have passed chkuser, simscan, clamav, and
spamassassin. Where else could this rejection be coming from? I'm
undoubtedly missing something. Anyone want to clue me in?
