QMT-ISO is an installation CD. When you boot off of it and hit enter at the first screen it will go through and format your hard drive. It will then install Qmailtoaster for you. That is stated on the site. It is meant for people installing Qmailtoaster for the first time that want everything done for them - no need to download the packages and run an installation script. It's included in the ISO.
