I can't think of another scenario off hand that would create this error.

The only way I know of to really know what's happening would be to look at a
detailed log. You can use recordio to obtain the details, but spamdyke has a
much nicer detail logging facility. Spamdyke splits each message's log into
its own file, so detail logs are much easier to evaluate and manage.
