Thanks for the reply. I found that a script had been setup on this
machine to do an rsync with a "delete-after" parameter and included in
this was the "/var/qmail" directory. When this script ran it was trying to
delete this directory while it was in use. I have changed the script and
all is well.

/var/qmail/supervise/spamd directory and "qmailctl stat" showed spamd
running correctly. After about a minute this directory was rename to
/var/qmail/supervise/spamd~ and I get the errors again.

So what is different with your installation that deviates it from the
standard? Do you have a script or something running somewhere?
