I learn something new every day. I guess qmail-inject *is* used
regularly in the send/delivery process.
# qmlog -lc inject send
shows entries such as:

Pretty clearly malformed stuff, and nothing to be concerned about.

would be my guess.

The interesting thing is that I always have that when mails seem to have multiple
recipients and one of them is clearly fake:<[email protected]> is not one
of my users while<[email protected]> is.
I also attached an error message that goes with such errors.
Please note that I have never yet seen this if it was not a spam mail anyway,
so I never investigated further.

Need a bit more information than that.
Which log?
Where's the message originating from?
I don't believe that qmail-inject is is typically used in a stock QMT
setting. Do you know what's invoking it?
Also, so we know at least a little about your environment, please post:
# qtp-whatami
