I believe the qq soft rejects are only occurring when clamd is
restarting. Is clamd supposed to be restarting? If not how do we fix
this.

it looks like clamd is restarting.

I'm seeing clamd restarting periodically in my QMT as well. It
appears that the sanesecurity update is triggering that.
I'm not getting any qq soft rejects though, so I expect the rejects
are not related to clamav restarting. Are your rejections happening
all the time, or just occasionally?
The qq soft reject can be the result of a number of problems,
usually scanning or queue related. I'd try stopping qmail, and run
"queue_repair.py -r" to see if that clears things up.
