To begin with, I don't know the answers to your specific questions. It
sounds as though qmail-pop3d is having a problem keeping track of what's
been downloaded.
I would recommend switching to dovecot for pop3. While I don't know if
dovecot's pop3 can restrict based on a number of days, I expect that it
would be much more reliable in its operation. I think that switching
from qmail-pop3d to dovecot pop3 will be pretty straight forward,
especially since you're already running dovecot for IMAP.
Someone else here might have more insight into this. Anyone?
FWIW, QMT will be using dovecot for imap and pop3 in a future release,
but I can't say when to expect this to happen. By migrating to dovecot
now, you'll be ahead of the game.
