The permissions of the files all look ok. I've looked at the C source
code, and it appears that if there's a problem, it should be showing a
message somewhere. I'm wonder if perhaps it's not getting to that point
in the code.
Anyone have any input on this?

qmailadmin-toaster-1.2.16-1.4.1.src.rpm is now available, which fixes
this problem. There was an error in the upstream code which blocked the
catchall functions from being processed (missing config.h include in
command.c module). I've reported it upstream, but haven't heard anything
about it.
On a side note, it'd be really nice to get qmailadmin redone to use
vpopmaild. Same for vqadmin. These aren't "core" components, and should
be able to be configured on a separate host. Then there would be no need
for apache to run on the same host as the mail server (although it still
could be if one chooses to). Anywise, that's a discussion for the
development list.
