If you just follow the qmail toaster instructions online, it explains everything you're asking for step by step on how to set up. And the qmt scripts do an awesome job of automating most of the install for you.
Qmail includes SMTP, so I'm unsure what you're asking for there. Personally, I'd recommend using roundcube instead of squirrelmail. You can google instructions on how to do it and if I'm not mistaken, qmt may already have a scripted procedure for setting up roundcube. I forget now how I got mine setup.
