Looks good! I fixed one typo (associated vs associative) but I think that
document now provides the info a server writer would need to implement the
extension.
BTW, for my own edification (and as part of a homework assignment), I spent
yesterday writing a new backend in Java. I don't know if mine will ever see
the light of day in qooxdoo, but it's not out of the question. I might then
consider adding some of these extensions.

Yes. It has been envisioned since the beginning, thus the PermissionDenied
error code, but never really thought out yet or formally implemented.
