I have been running qpsmtpd for a while (mostly due to wanting the sender_permitted_from support), and I have a few patches that might be of interest:
1) Support sending responses where there are embedded newlines in the response message (and the plugin hasn't split correctly)
2) Add support for dnsbl_allow to the dnsbl plugin. This allows you to nominate a list of IP addresses that are not not subject to RBL filtering.
3) Add customized reject message on dnsbl failure (from dnsbl_rejectmsg)
4) In sender_permitted_from, fix a crash when sending mail to postmaster.
5) In sender_permitted_from, fix a problem when mail is being sent to you from your MX secondary and you (incorrectly) deny it due to SPF filtering.
