It's a nice short form, but doesn't really go into much detail. I'm using a mixture of Qt3 and git master-stable, and using it for a mix of network daemons and client-server code, for instance, which doesn't really map onto the multiple choice options. You might get some extra useful info if you add some deeper (maybe optional) questions about what people are using and how along with the next survey, maybe.
